Our internal auditors noted two errors in board minutes that had already been approved by the board - (1) wrong name of officer who was granted options (we incorrectly had the name of a former officer in the minutes) and (2) sum of annual bonus amounts had typo so numbers reversed.

What is best practice to revise minutes after they have been approved by the board?

Usually in that circumstances I would submit a resolution to the Board amending the minutes.
