We want to develop a Board post-orientation survey to assess the effectiveness of our Board orientation process. Would anyone have implemented such survey and would be willing to share questions?

We don't have such a survey. Rather we just ask new Directors to share their feedback (either orally or in writing).

However, I would think identify the objectives of the orientation and then creating some questions around how well those objectives have been met would be one approach you could take.

New board members are asked to provide feedback about our on-boarding program after they have attended two regular board meetings. The feedback has confirmed that this interval allows them to have enough experience of our board to assess the effectiveness of the on-boarding program and provide suggestions of ways to improve it.
